Social prescribing is a way to address people’s non-medical needs holistically, empowering individuals to take greater control of their own health. The Digital First Social Prescribing Programme aims to enhance how social prescribing is provided by digitising key elements of the resident’s journey and to improve the experience of all those involved in social prescribing:
- residents accessing social prescribing
- health and care professionals
- social prescribers
- providers of community activities
The Barnet pilot will identify the key functions of social prescribing case management software which is required to meet the needs of everyone involved in social prescribing. The Voluntary and Community Sector in Barnet are a key stakeholder group and we need your views to inform this vital work.
